If you want everyone to be nude, and the player is the only one in underwear, the best way to do it is to make a custom player race that uses a unique body mesh and texture with underwear. This is possible, and (at least in FO3, FNV, and Skyrim) pretty easy. No, it would just take a reskin with no underwear drawn on it.

- For male bodies for New Vegas, I use Breeze mainly because there are more armors available for that body. Apparently, in Fallout 3, clothing and armor for Breeze would also work for Roberts. That is not the case in New Vegas.
